Metal on Metal Hip implants/PIP breast implants – Monday 30 January 2012

We are currently receiving a high volume of enquiries about metal on metal hip implants and Poly Implant Prosthese (PIP) breast implants.

Any patient requiring specific information about their implants is asked to email their request to enquiries@gjnh.scot.nhs.uk.

When emailing, please provide the following information to help retrieve your medical records and answer your enquiry more quickly.

Address (if different from current address, please provide the address you lived at when you had the procedure)

Community Health Index (CHI) number (if known)
Approx. date of your procedure (if known)
Any other relevant information (such as surgeon name etc)
